Notification of Collection of Personal Health Information

When you receive health services of any kind from Blair, Gripp,  Stubbs and Associates Radiology Inc., some personal information needs to be shared with your caregiver so that the right care or treatment can be provided.

The personal information that you are asked to give is collected, used and disclosed in accordance with the provisions of the Health Information Act (HIA). As well, the access and privacy provisions of the legislation require that Blair, Gripp, Stubbs and Associates Radiology Inc. protect your personal health information from unauthorized access, collection, use, disclosure or destruction.

Digital Imaging Services Notification

When you receive imaging services of any kind from Blair, Gripp, Stubbs and Associates Radiology Inc., your health care provider, via a secure, virtual private network, can access your images. We are pleased to offer this service in an effort to provide you with a timely diagnostic service.

Please be assured we have taken all measures necessary to ensure your health care provider is the only one who can access your images remotely.
